Six week programme providing help to people living with a life-limiting condition such as cancer, COPD, or heart failure, where no more treatment is available or desired. It includes tailored exercise sessions, relaxation techniques, occupational therapy, benefits advice, mindful eating advice and help with regulating emotions. See website for more information.

People living with a life-limiting condition. Referrals are by a health care professional. Self-referrals also accepted if the person has been diagnosed or is receiving treatment for either cancer or another long term condition
